Deep tissue has been found to improve muscle function and range of motion, break up scar tissue and adhesions, and decrease pain. It is thought that when muscles are tensed, they block oxygen and nutrients, leading to inflammation that builds up toxins in the muscle tissue.  A deep tissue massage may help loosen muscle tissues, release toxins from muscles and get blood and oxygen circulating properly. Because many toxins are released, it is recommended to drink plenty of water after a deep tissue session to help eliminate these toxins from the body.
If you are a massage therapist, or sports physical therapist, it might be a good idea to explain what a this type of massage will accomplish and what to expect.  At first, a Deep Tissue massage might feel like your typical Swedish massage. First, your therapist will warm up and prepare your muscles by applying light pressure to the areas that require attention. Sports massage was developed to help athletes prepare their bodies for optimal performance, recover after a big event, or function well during training. However, contrary to what the name suggests, you don't have to be an athlete to benefit from sports massage. Sports massage emphasizes prevention and healing of injuries to the muscles and tendons and can be beneficial for people with injuries, chronic pain or restricted range of motion.
Further interest in fascia is now evident. Fascia is a relative newcomer to the neuro-muscular-orthopaedic concept. Often misunderstood and ignored, fascia has an important role in structure, function and movement etc. Travell discussed the role of fascia in referred pain in 1952 in the paper "Pain mechanisms in connective tissues" (see reference: Travell JG, Office Hours day and Night, 1969, chapter 14, page 257, The world Publishing Company. The first world Fascia Research Congress will commence in Harvard, Boston in October 2007 and will bring together researchers and clinicians of many disciplines to integrate the current research on fascia in form and function. Massage and myofascial therapies should aim to influence the research direction and be involved from the outset.
Some strokes massage therapists use in deep tissue massage include kneading, tapping, circular movements, and elongated movements, and they may use elbows and forearms for increased pressure in addition to knuckles and fingertips. Many people confuse reflexology with massage, Reiki, or acupuncture, but there are essential differences between these therapies.  Massage therapists manipulate larger areas of soft tissue in the body while reflexologists apply pressure to specific points on the feet, hands, and ears.  Unlike either massage or reflexology, Reiki does not involve any physical manipulation or pressure, but instead uses light touch to work with the subtle vibrational field thought to surround the body. Finally, while acupuncture and acupressure, like reflexology, use reflex points on the body to influence other parts of the body, the points are not the same and acupuncture uses points over the entire body.

If massage can “improve” any tissue — unknown — one way it might do it is through simple hydraulics: physically pumping tissue fluids around, and/or stimulating the circulation of blood and lymph. I won’t get into the evidence about it here. Suffice it to say that it might be true, and if it’s true then it may not much matter if the process is uncomfortable. While gentler massage may feel pleasant and satisfying, it is possible that more biological benefits can only be achieved hydraulically — whether it’s comfortable or not. This is even more plausible because of trigger points: it’s likely that the tissue fluids of a trigger point are quite polluted with waste metabolites, and the need for flushing is greater, but it’s especially uncomfortable to squish those polluted patches of tissue.

One risk is clearly neurological and complex: some people are basically sitting ducks for the well-documented and nasty phenomenon of “central sensitization,” and indeed may already be in pain and seeking help because of it. A strong massage can severely aggravate that situation, with long term and extremely unfortunate consequences. It’s rare, but it happens. The typical clinical scenario here is a gung-ho under-trained therapist over-treating someone in, say, the early stages of fibromyalgia. Bad, bad, bad.

DTM can be useful to those that are recovering from an injury (once the client/patient is out of the acute phase), for athletes, for people with postural strains, or people with chronic pain. Typically there is an area or a few areas where this type of work is needed. For example, a person who has chronic postural pain/tightness from sitting at a computer, might need DTM to their shoulders, chest, and upper back/neck. They likely, do not need DTM on their whole body. Some therapists might disagree with me here, but I rarely think a full-body, DTM, is needed. It can simply be too much. I would rather see a client more often, for less-intense sessions. It is simply more effective. It is the same as Physical Therapy- it is more effective to do it regularly.

Both Thai and  Swedish massages are reported to relieve chronic low back pain by enhancing physical functions;  providing pain relief, improving disability and range of motion, improving psychological functions;  reducing anxiety and improving mood. Although based on different theoretical frameworks, they appear to be equally effective in relieving chronic low back pain.
