Some reflexologists who deny that they diagnose or treat disease claim that the majority of health problems are stress-related and that they can help people by relieving the "stress" associated with various diseases or body organs [1]. This type of double-talk is similar to chiropractic claims that "subluxations" lower resistance to disease and that "adjusting" the spine to correct subluxations will improve health. All ten of the books I have inspected mention scores of health problems that reflexology has supposedly helped.

No one really knows how a painful massage can also feel so good at the same time. This is a sensory phenomenon mostly beyond the reach of science — not entirely14 — all we can do is speculate. A main question is whether good pain is good because we expect relief to follow pain, or because positive and negative qualities are being produced simultaneously. My bet is on the latter.

No. Bottom line, massage should never hurt if you don’t want it to. Some clients specifically say that they do not want to be in pain, and that should be respected. However, there are certain techniques that might cause discomfort. If the client and therapist communicate and agree on increased pressure, you can incorporate these deeper or more aggressive techniques into the massage. They can cause a little pain at the time, and a little bit of soreness the next day. I like to compare it to how you feel after a good workout. A good Massage Therapist will also be very skilled at warming and softening the tissue layer by layer to decrease the amount of pain felt by the client.

Connective tissue stimulation. A lot of therapists are keen on stretching connective tissues — tendons, ligaments, and layers of Saran wrap-like tissue called “fascia.” I’m not a huge fan of this style, but certainly it’s a way of generating many potent and novel sensations, which may be inherently valuable to us — another form of touch. Although “improving” the fascia itself is implausible and unproven, perhaps fascial manipulations affect bodies indirectly, just as a sailboat is affected by pulling on its rigging. People have written whole books full of speculation along these lines. So, as long as the sensations are not like skin tearing (that’s an ugly pain for sure), you might choose to tolerate this kind of massage if it seems to be helping you.
Until better evidence is published in peer reviewed journals, he remains skeptical of claims that by massaging or applying pressure to specific points on the hands or feet, a reflexologist can alleviate problems in corresponding organs or other systems throughout the body. He has seen no evidence showing that reflexology is effective for pain or any health problems unrelated to the feet and hands. He urges caution with regard to claims that reflexology can cleanse the body of toxins, increase circulation, promote weight loss, or successfully treat earaches, hemorrhoids, emphysema, heart disease, thyroid disorders or any other health condition.
Prenatal massage uses mild pressure similar to Swedish massage. The therapist will focus on areas such as your lower back, hips, and legs. You can be fully or partially undressed depending on your comfort level. During the massage, you’ll either lie on your side or on a specially designed table with a cutout for your belly. If you’ve had pain in your calves or other parts of your leg, see a doctor before you have a massage.

First, you should know that feeling some levels of pain is good sign. This means that your therapist is breaking up those knots and realigning stiff muscles. However, if the pain becomes too intense or excruciating, you should always notify your massage therapist. Our therapists are trained to deal will all levels of pain sensitivity and will work to approach your particular situation using another technique or work to further prep your muscles.

So what should runners book instead? Anna Gammal, a massage therapist who works with elite runners at the Boston Marathon each year and also massaged athletes at the 2004 and 2012 Olympics, recommends either a sports massage (i.e. targeted therapeutic treatment for the unique physical and biomechanical needs of athletes) or a myofascial release massage (i.e. the application of gentle, sustained pressure on soft tissue restrictions). Both specifically target muscle release and will help improve flexibility, reduce pain and increase range of motion.
Great blog on deep tissue massage! As a massage therapist I can relate to all of this. Clients don’t really know what they are asking for when they want deep tissue. I have to break in down in easy terms for them to understand. On many when I have tried to do deep tissue they say ok too deep lighten up lol! I tell them that they aren’t ready for deep tissue. I find deep tissue massage is better geared to athletes and gym goers 🙂
Neck pain. Neck pain is tied with headaches as the second most common pain experienced by adult Americans (also 15 percent), and massage can typically help with this too. For instance, a systematic review and meta-analysis published in 2014 in Evidence-Based Complementary and Alternative Medicine analyzed 15 studies and found that there was “moderate evidence” that massage therapy helped provide relief.
According to the National Center for Complementary and Integrative Health (NCCIH), this includes individuals with bleeding disorders, low blood platelet counts, or those who are taking blood thinning medications. When these types of conditions are present, the NCCIH indicates that a sports massage with deep tissue work is generally not recommended.

Thai massage, also called Thai yoga massage, combines acupressure and assisted yoga postures. Thai massage is based on the flow of energy through the body and focuses on energy lines called “sen.” It doesn’t use oils or lotions and, instead of rubbing muscles, the massage therapist compresses, pulls, stretches and rocks the recipient’s body to promote the flow of energy through these sen. In this form of massage therapy, practitioners use their hands, knees, legs and feet to apply deep muscle compression and stretch the body. It’s often performed on a mat on the floor instead of on a massage table. Like most forms of massage, Thai massage is used to relieve stress, improve range of motion and enhance flexibility. Ugly pain in massage therapy is, by my definition, never okay. Ugly pain is often caused by things that are not going to offer even minimal, delayed benefits, and may even be dangerous. It’s important to be able to spot ugly pain for what it is and completely eliminate it from any therapy you’re receiving. What kinds of handling may cause “ugly” pain?
A study conducted by the National Center for Complementary and Alternative Medicine, and published in The New York Times, found that volunteers who received a 45-minute Swedish massage experienced significant decreases in levels of the stress hormone cortisol, as well as arginine vasopressin-a hormone that can lead to increases in cortisol. There is no consensus among reflexologists on how reflexology is supposed to work; a unifying theme is the idea that areas on the foot correspond to areas of the body, and that by manipulating these one can improve health through one's qi.[8] Reflexologists divide the body into ten equal vertical zones, five on the right and five on the left.[9] Concerns have been raised by medical professionals that treating potentially serious illnesses with reflexology, which has no proven efficacy, could delay the seeking of appropriate medical treatment.[10]

Effleurage is the most common stroke in Swedish massage. It is a free-flowing and gliding movement towards the heart, tracing the contours of the body using the palm of one or both hands. Oil is applied with this stroke to begin the first stage of massage. The therapist applies a light or medium constant pressure. This stroke is used to warm up the muscles, relax the body, calm the nerves, improve blood circulation and heart function, and improve lymphatic drainage.

Some research has suggested that it may actually be possible to physically destroy the motor end plate with strong massage, thereby inactivating the trigger point.10 When it regrows — these are microscopic structures, it doesn’t take them long to heal — the trigger point may be gone.
Since reflexology is not recognized by law, no formal training is required to practice reflexology or call oneself a reflexologist. However, some nurses and massage therapists offer reflexology as part of their licensed practice. Some courses are accredited for continuing education for nurses and massage therapists. The most widely publicized training source is probably the International Institute of Reflexology, of St. Petersburg, Florida, which claims to have 25,000 members worldwide [9]. Its seminar on the "Original Ingham Method of Foot Reflexology" are taught by Ingham's nephew, Dwight Byers. Its "Certified Member" status requires 200 hours of instruction plus passage of written and practical tests. As far as I know, this certification process has neither legal nor medical recognition. The Institute's Web site states:
Pain management. If you have a condition like sciatica or osteoarthritis and are suffering from chronic pain as a result, Swedish massage can be an effective method for managing that pain in a natural way. Notify your massage therapist about your pain points, he or she can target those areas and use a stroking motion to improve local circulation and reduce muscle tension.

There is not necessarily a hard line between these two techniques, and many sessions often incorporate both depending on your needs. It is usually the case that not all the muscles in your body need deep tissue techniques applied. Rather than being overly concerned with choosing the “right” session, make sure you communicate to your therapist the goals for your session so that he or she can customize the right blend of techniques for you. One massage style is often the foundation of the session, with other techniques used as needed. Due to the slow pace of deep tissue massage it is necessary to schedule a 90-minute session if you would like your full body addressed. Either it isn’t literally painful (just intense), or it’s painful but desired anyway because of relief or belief: an actual biological relief or at least the belief that there is one. There are five main techniques in Swedish massage: effleurage, friction, petrissage, tapotement and vibration. Within each category there are various movements, all intended to ease muscle tension and pain. Each movement has a specific purpose for loosening, warming, soothing or stimulating your muscles during your massage. Here is a breakdown of each of the five Swedish massage techniques:
Injury: in the case of an injury, the recovery treatment will adapt to the healing process of the injury. At the beginning of an injury, massages are more frequent, short and focused on the area. For example, a sprained ankle may need light but bi-weekly work after the acute phase is over. As the injury recovers, massages are more intense, and less frequent. The ankle will receive deeper massages and deeper stretches as it heals. Once the injury is recovered, only one or two check-up massage sessions will be required.
Traditionally designed to apply pressure to layers of muscle, tendons and other tissues deep under the skin, deep tissue massage can be very effective in relieving problem areas and is highly therapeutic. Deep tissue massage is generally a modality to consider for athletes as well as the eight-to-five desk job. Muscles easily become tense and deep tissue massage can relieve some chronic patterns of pain and tension, specifically in the back, shoulder and neck areas.  
Sports massage was developed to help athletes prepare their bodies for optimal performance, recover after a big event, or function well during training. However, contrary to what the name suggests, you don't have to be an athlete to benefit from sports massage. Sports massage emphasizes prevention and healing of injuries to the muscles and tendons and can be beneficial for people with injuries, chronic pain or restricted range of motion.
