“Good pain” is at the heart of the pressure question: a strange, potent sensory paradox that many people actually seek out as the goal of therapy, consciously or unconciously. The American Reflexology Certification Board (ARCB) has a three-part examination process to ensure the practitioner has met the standards set by the board. In order to be certified through ARCB, a minimum of 110 hands-on training hours must be completed.

Now for some benefits that are not supported by research. The ability of sports massage to help the muscles get rid of lactic acid is not supported in research studies. Many researchers feel this is linked to the fact that sports massage does not increase blood flow to muscles. There are many different styles and types of massage. Two of the most common methods of massage are Swedish massage and deep tissue massage. Before you meet with your massage therapist, it is helpful to know difference between the two. When you have a good understanding of both you can explain exactly what type of massage you want and what your goals are for your massage.
Referred pain spreads the goodness. Undoubtedly another reason that massage pain can feel good is the phenomenon of referred sensation. If you stimulate internal tissues anywhere in the body, muscle or otherwise, the brain really has trouble telling quite where the sensation is coming from. When you press hard enough on your muscles, particularly on sensitive trigger points, the pain is often experienced as though it originated from a much broader area.

Plantar fasciitis. According to the JAMA Network, plantar fasciitis occurs when the fibrous band on the bottom of the foot becomes irritated and inflamed, causing pain in the heel and arch areas. The Pacific College of Oriental Medicine further indicates that deep tissue massage can be an effective treatment for this particular condition as it helps “release the muscle tension, break scar tissue, and lead to its elimination.”

Traditional Thai massage is a different experience if you’ve only ever had a Swedish or deep tissue massage. In Thai massage, you wear loose-fitting clothing (often provided by the massage therapist) and start by lying on a mat. Throughout the massage, the therapist moves your body into various seated and prone positions, which stretch and release your muscles and soft tissue. Although your body is moving, the therapist is doing all the work, making it feel similar to supported yoga. The massage therapist will bend, stretch, and compress and lengthen your body using their hands, forearms, elbows and even feet. Don’t be alarmed by the feet! Many studios have overhead supports that allow the practitioners to safely walk on your back, releasing tight adhesions and promoting ease. Thai massage can be both invigorating and relaxing. It is an excellent option for keeping your muscles in good health if you regularly work out, or if you want to overcome a lack of flexibility. Studies show that Thai massage can increase blood circulation, improve flexibility, reduce muscle tension and enhance mental well-being.
Massage should not be done directly over bruises, inflamed or infected skin, skin rashes, unhealed or open wounds, tumors, abdominal hernia, fragile bones, or areas of recent fractures. Massage may cause bruising and rarely, hematoma (a localized collection of blood outside of blood cells), venous thromboembolism, and a condition known as spinal accessory neuropathy.
Deep-tissue massage can be an effective treatment for injured muscles. Because it facilitates the movement of toxins from the muscles and helps stretch tight or twisted muscle mass, deep-tissue massage can help promote healing. Because massage also helps relax muscles, it can reduce the pain caused by injuries, too. Deep-tissue massage is frequently used to rehabilitate sports injuries.
Swedish massage therapy is the modality that comes to mind when most people think about massage. As the best-known type of bodywork performed today, one of the primary goals of the Swedish massage technique is to relax the entire body. This is accomplished by rubbing the muscles with long gliding strokes in the direction of blood returning to the heart. But Swedish massage therapy goes beyond relaxation. Swedish massage is exceptionally beneficial for increasing the level of oxygen in the blood, decreasing muscle toxins, improving circulation and flexibility while easing tension.

Swedish massage is the most common massage therapy technique in the United States. (In case you were wondering, Swedish massage is called “classic massage” in Sweden.)   A Swedish massage focuses on overall relaxation, circulation, and physical and mental wellness.  Swedish massage includes gliding, kneading, tapping, stretching, and cross-friction strokes.
Flushing. If massage can “improve” any tissue — unknown — one way it might do it is through simple hydraulics: physically pumping tissue fluids around, and/or stimulating the circulation of blood and lymph. I won’t get into the evidence about it here. Suffice it to say that it might be true, and if it’s true then it may not much matter if the process is uncomfortable. While gentler massage may feel pleasant and satisfying, it is possible that more biological benefits can only be achieved hydraulically — whether it’s comfortable or not. This is even more plausible because of trigger points: it’s likely that the tissue fluids of a trigger point are quite polluted with waste metabolites, and the need for flushing is greater, but it’s especially uncomfortable to squish those polluted patches of tissue.

The effects and benefits of Swedish Massage have been well researched and documented with controlled studies.  Research performed by the Touch Research Institute at the University of Miami School of Medicine and International Journal of Neuroscience shows that 45-minute massage increases serotonin, dopamine and oxytocin that help to balance and elevate the mood.  Serotonin and dopamine are both neurotransmitters.  Lowered levels of either serotonin and dopamine have been linked with depression, anxiety and overall lethargy.  Increased levels,  as with massage, will bring about a feeling of emotional well-being and balance. Oxytocin, commonly referred to as the “cuddle hormone,” has been shown to release while cuddling, as well as while in massage.  While it is technically a hormone, it tends to act like a neurotransmitter making a sweet little neuropeptide that makes you feel warm and soft on the inside.  


Shoulder pain. A 2017 study published in the Journal of Physical Therapy Science reports that shoulder pain is one of the most common musculoskeletal disorders, affecting as many as 66.7 percent of the population. The study goes on to say that massage therapy can often help reduce this type of pain, sometimes in a matter of days, though 36 sessions appeared to offer the greatest level of relief.
If you are a massage therapist, or sports physical therapist, it might be a good idea to explain what a this type of massage will accomplish and what to expect.  At first, a Deep Tissue massage might feel like your typical Swedish massage. First, your therapist will warm up and prepare your muscles by applying light pressure to the areas that require attention. Only after your muscles have been sufficiently prepared will your therapist begin applying specific techniques. The most commonly used strokes in Deep Tissue massages are stripping and friction. Stripping usually involves your therapist applying deep and gliding pressure to the length of your muscle fibers with an elbow, forearm, knuckles or thumbs. Friction, on the other hand, applies pressure across the grain of the muscle in order to relieve adhesions and realign the fibers of the tissue.
To understand this difference, it’s helpful to first think of the body’s fascia and muscles in layers. Notice in this image the many overlappinglayers of these tissues. Fascia is a connective tissue which permeates the entire body – literally holding the body together, wrapping around every muscle, nerve, organ, blood vessel, and bone. These wrappings are all interconnected in a three-dimensional maze. The muscle layers run superficial to deep in the body.

One risk is clearly neurological and complex: some people are basically sitting ducks for the well-documented and nasty phenomenon of “central sensitization,” and indeed may already be in pain and seeking help because of it. A strong massage can severely aggravate that situation, with long term and extremely unfortunate consequences. It’s rare, but it happens. The typical clinical scenario here is a gung-ho under-trained therapist over-treating someone in, say, the early stages of fibromyalgia. Bad, bad, bad.

Harriet Hall, MD also known as The SkepDoc, is a retired family physician who writes about pseudoscience and questionable medical practices. Good pain. In massage, there is a curious phenomenon widely known as “good pain.” It arises from a sensory contradiction between the sensitivity to pressure and the “instinctive” sense that the pressure is also a source of relief. So pressure can be an intense sensation that just feels right somehow. It’s strong, but it’s welcome. Good pains are usually dull and aching, and are often described as a “sweet” aching. The best good pain may be such a relief that “pain” isn’t even really the right word.
